Shift handovers are a high-risk information boundary. Safety issues, service position, staffing gaps, equipment problems, quality exceptions, priorities and open actions can be lost or interpreted differently when the transfer depends on memory or an unstructured conversation.
What I designed
a consistent handover structure for safety and service position
staffing and equipment status visibility
quality and exception handoff
explicit priority and open-action transfer
clear ownership of what the incoming shift needs to pick up
How I approached it
I treated the handover as a repeatable knowledge-transfer process rather than a note-taking exercise. The structure makes the expected information categories visible and gives the incoming shift a consistent control point for what has changed, what remains open and what needs immediate attention.
