Case management built around how clinicians actually work

Paloma Health's clinical team was stuck using a third-party case management platform that didn't match how they actually worked. It was clunky, bloated, and getting in the way of the people using it every day. They needed something custom built around their real workflows.
I started with discovery to understand the business, then spoke directly with stakeholders to map out the current platform's limitations. User interviews gave me first-hand insight into the daily frustrations. Together, that research surfaced the core themes that became the foundation for the new system.
From there I moved into wireframes, tested them with real users, and refined the UI based on their feedback. Everything was built on a custom Shadcn design system to keep handover clean and support the dev team through implementation.

The Outcome

The MVP has been built and is now live with real users. Early feedback from clinicians has been really positive. The system fits how they actually work, which was the whole point. Case management is faster, less frustrating, and the team is spending less time fighting their tools and more time focused on patient care. The response has validated the research and design decisions made early on, and the platform is set up to evolve as the team's needs grow.
