The Stakes

Immunization tracking isn't a nice-to-have. Missed vaccines mean real health consequences for children. The client needed a centralized digital registry that could replace fragmented paper records across hospitals and clinics, ensuring no child falls through the cracks.
The system had to handle child registration, vaccine scheduling, certificate generation, hospital selection, and automated reminders. And because it deals with children's medical data, security and data integrity were non-negotiable.

Understanding the Workflow

I spent the first month mapping the immunization pipeline end to end. How are children registered? Which vaccines are given at which ages? How do parents choose between hospitals and clinics? What happens when a family moves to a different city? How are records verified?
The research surfaced a critical design decision: the system needed to work for two very different user groups. Parents need a simple, reassuring interface to track their child's vaccinations. Health administrators need detailed data views, reporting tools, and the ability to manage records across multiple facilities.
I developed 3 design concepts that balanced these needs differently. The approved direction gave parents a clean, focused experience while keeping the admin tools powerful but tucked behind role-based access.

The Platform in Detail

The main dashboard surfaces what matters most: upcoming vaccinations, overdue records, and facility-wide coverage stats. Administrators can see at a glance how many children are on schedule, how many are overdue, and which vaccines have the lowest completion rates. It's the early warning system for the entire registry.

Every child gets a comprehensive digital profile from the moment they're registered. Date of birth, guardian information, medical notes, and a complete vaccination timeline are all stored in one place. The record follows the child regardless of which facility they visit.
Vaccination Schedule
Vaccination Schedule
The vaccination schedule is generated automatically based on the child's age and national immunization guidelines. Each vaccine shows its recommended window, current status (completed, upcoming, or overdue), and the facility where it was administered. Parents can see exactly what's next without deciphering medical jargon.
Hospital Selection
Hospital Selection
Parents can browse and select from registered hospitals and clinics in their area. Each facility shows its available vaccines, operating hours, and location. This gives families flexibility while keeping all records centralized in the registry regardless of where the vaccination happens.
Immunization Certificate

Digital immunization certificates are generated automatically after each verified vaccination. These certificates include the child's details, vaccine type, batch number, administering facility, and date. They can be downloaded as PDFs for school enrollment or travel documentation.
Admin Panel

The admin panel gives health officials control over the entire registry. They can manage facility registrations, review flagged records, update vaccine inventories, and oversee data quality across the system. Every administrative action is logged for accountability.

The reporting module turns raw vaccination data into actionable insights. Coverage rates by region, vaccine type, age group, and time period are all visualizable. These reports help health authorities identify underserved areas and allocate resources where they're needed most.

Automated reminders are the safety net that prevents missed vaccinations. Parents receive notifications before upcoming vaccine dates and escalating alerts when a vaccination becomes overdue. The system tracks reminder delivery and response rates so administrators can identify families that may need outreach.

The data management layer handles the complexity of medical records at scale. Bulk imports, record merging (for duplicate entries), data validation rules, and export capabilities are all built in. The system enforces data integrity constraints to prevent incomplete or contradictory records.

Child registration is designed to be completed in under 2 minutes. The form collects only essential information upfront: child's name, date of birth, guardian details, and preferred facility. Additional medical history can be added later. The low barrier to entry was intentional to maximize registration rates.
User Management
User Management
User roles are tightly controlled given the sensitivity of the data. Parents see only their own children's records. Facility staff see patients registered at their location. Regional administrators see aggregate data for their jurisdiction. System administrators have full access with complete audit logging.
