For Tiara Time Spa, created a website on a WordPress platform using Elementor and Royal Addons. The website had a form that automatically mailed the business founder of services requests, but since they had issues with their email system, I suggested we create a form customers can print out and send via email. This solutions seems to be working at the moment. The form was around two pages, and it required to have various required fields to be checked, so I did the best to fit it into one page with all the fields the founder required of the customer.
