About this product

GIC are the gulf between the recommended treatment trajectory for a certain patient and the treatment that patient actually gets. They can manifest in several ways, the most common of which includes Individuals who are missing age-based or annual screenings. This product helps Care Coordinators in call centers to schedule appointments faster, so more patients can start their preventive care journey.
By providing better care to more people early on, closing care gaps improves both health and economic outcomes, in addition to helping improve health equity.

I designed this product while working as Design Lead at ZL technologies.

Workflow-Create an appoinment

3 easy steps to schedule an appoitment to close a gap in care. The user is a Care Navigator, who uses this app from his/her computer in a call center, while talking on the phone with a patient.

Example-Insurance Company

This Insurance Company is an important client that uses the App for Closing Gaps in Care. They specifically use it for making appointments from the point of visit (specialized nurse meeting a Medicare patient at his home). This is a prototype built for them used in a sales meeting.
