Went with the confident doctor portrait, it reads more reassuring to me than the caretaker scene. Warm photography and soft colors are doing a lot of the trust-building work here without tipping into sterile.
Option 2 for me. It feels more human and trust-building because it shows the care experience, not just the doctor. Option 1 is cleaner, but 2 matches the brand message better.
