About the Project:

TriageHome – Practitioner is a mobile app designed for clinical practitioners to deliver professional healthcare services directly to patients in their homes.
The platform connects verified clinical practitioners such as doctors, nurses, and therapists with patients who need convenient, at-home medical care.
Through the app, practitioners can receive appointment requests, manage their schedules, document patient care, and track their earnings, all in one place.

The Challenges:

TriageHome – Practitioner aims to empower clinical practitioners to deliver quality healthcare services seamlessly outside traditional clinical settings. So I created solutions for the challenges below:
Clinical practitioners often lack structured platforms to easily receive and manage home visit requests, leading to missed opportunities and inefficiencies.
Managing appointments manually or across multiple channels can be disorganized, making it difficult to keep track of schedules, locations, and patient details.
There is limited access to reliable systems that allow practitioners to securely document patient interactions, clinical notes, and medical records after home visits.
Trust and verification are critical, as practitioners need a platform that showcases their credentials and builds confidence with patients before a visit.
Many practitioners face delays or lack transparency in payment processes, making it harder to track earnings and maintain financial clarity.
Handling recurring or multiple appointments for ongoing patient care is often complex and not well supported by existing tools.
Practitioners need a simple and efficient workflow, especially when attending multiple home visits in a day, but many systems are time-consuming and difficult to use.
Access to patient information before and after visits is often limited or fragmented, reducing the quality and continuity of care.
There is a lack of centralized communication and updates, making it harder for practitioners to stay informed about new requests, schedule changes, or cancellations.
Healthcare tools for practitioners often lack intuitive design, making it difficult to quickly navigate, update records, or complete tasks while on the move.

My Approach:
To address these challenges, I focused on designing an experience that is efficient, structured, and practitioner-first, while supporting clinical practitioners who are often on the move and managing multiple responsibilities:
I simplified complex workflows such as managing appointments, documenting visits, and updating records into clear, step-by-step actions. Each screen was designed to help practitioners complete tasks quickly without unnecessary friction.
I prioritised clarity and speed by structuring the app around key actions like accepting requests, managing schedules, updating medical records, and tracking earnings. This ensured practitioners could focus on delivering care rather than navigating the app.
I designed the appointment system to support both single and recurring visits, making it easier for practitioners to manage ongoing patient care without confusion or manual tracking.
I created a structured medical records workflow that allows practitioners to easily document assessments, observations, and recommendations immediately after each visit, ensuring accuracy and continuity of care.
I ensured that critical information such as patient details, location, service type, and timing is always visible and accessible, helping practitioners prepare effectively before each visit.
I introduced clear status indicators and notifications to keep practitioners informed about new requests, schedule updates, and cancellations in real time.
I maintained consistency across screens such as appointments, records, and profile management so practitioners can quickly learn the system and navigate it effortlessly, even during busy schedules.
I designed the experience to be mobile-first and time-efficient, considering real-world scenarios where practitioners may be traveling between visits or working under time constraints.
I focused on building trust and professionalism by highlighting practitioner credentials, verification, and structured documentation, reinforcing confidence in the platform.

The Results:

I designed TriageHome – Practitioner to remove the common challenges clinical practitioners face when delivering care outside traditional settings, such as disorganized scheduling, difficulty managing patient records, and lack of clear systems for handling appointments and payments.
Instead, I created a streamlined platform that allows clinical practitioners to receive requests, manage their schedules, document patient care, and track their earnings with clarity and efficiency.
At its core, TriageHome – Practitioner is designed to give clinical practitioners structure, control, and flexibility, enabling them to focus on delivering quality care while the platform handles the complexity of coordination, documentation, and workflow management.
