Senior Care Connect — AI-Powered Care Coordination Platform

Overview

Senior Care Connect is an AI-powered care coordination platform built with Next.js and OpenAI. It connects seniors, family members, and professional caregivers in a unified dashboard for managing daily care, health monitoring, and emergency response.

The Problem

Families managing senior care face fragmented tools: medication reminders in one app, care notes in another, emergency contacts in a third. Caregivers lack a central view of patient status, and seniors have no easy way to signal distress. The result is delayed responses and burnout for families.

What I Built

Unified care dashboard: caregivers see all assigned seniors, health logs, and task queues in one view

AI-generated daily care summaries: OpenAI summarises activity logs and flags anomalies for family review

Medication reminder system: scheduled alerts with confirmation tracking and missed-dose escalation

Emergency alert flow: one-tap SOS from senior triggers SMS and in-app alerts to all care team members

Activity log with AI insights: pattern detection identifies sleep disruption, reduced mobility, or mood shifts

Multi-role access: separate views for senior, family member, and professional caregiver with appropriate permissions
