Project Overview

Working with a school district serving over 53,000 students, I partnered with the counseling department to standardize the intake protocol of their student suicide indicator and ensure each student received the same high level of care and attention. I utilized Qualtrics to power the survey because it allows the flexibility I needed to automate weekly student lists, email copies of reports, link follow-up resources via PDF, and export data directly into their student information system.

Key Responsibilities

Partner with school counseling to establish suicide assessment questions, format, and follow-up guidelines
Establish custom survey flow directing standards of care based on survey responses
Link follow-up PDF documents for counselors or school psychologists
Work with IMS to configure XM Directory contact import and map all external data fields
Establish data export timeline and test connection to the information system

Results

As a result of this project, 77 students were administered the intervention survey during the pilot. Counseling reported that the standard of care had increased due to the survey process which required them to complete each step. Information is logged into their information system daily, giving them a record that is tied to individual students and details about their visit including who administered the assessment, follow-up notes, and who was contacted.
