Cordia® is a digital health tech platform designed to support chronic disease management by connecting patients, clinicians, relatives, and administrators through a connected digital ecosystem.

The project covered the design of the Cordia marketing website alongside the core product ecosystem, including the patient mobile app, clinician web platform, relative app, and super admin portals.

The goal was to create a cohesive digital ecosystem that could support different stages of the Cordia experience, from discovering the platform through the marketing website to actively managing health through its products.

The product needed to make complex healthcare information easier to understand while giving each user role the tools and workflows relevant to them.

The project involved designing for multiple audiences and touchpoints while maintaining a consistent product language across the ecosystem.

The work focused on:

Understanding the needs and workflows of each user role.

Mapping journeys across patient, clinician, relative, and admin experiences.

Simplifying complex healthcare workflows and information.

Establishing reusable UI patterns across platforms.

Designing the marketing website around Cordia's value proposition and audience.

Creating clear connections between product capabilities and user needs.

Considering different states, permissions, and edge cases.

This helped establish a unified experience across both product and marketing touchpoints.

Designed the Cordia ecosystem across five key experiences:

Marketing Website: Designed a website that communicates Cordia's value proposition, explains its platform and capabilities, and introduces the different experiences available to patients, clinicians, and care teams.

Patient Mobile App: Designed experiences for health monitoring, questionnaires, health information, and day-to-day interactions with the platform.

Clinician Web Platform: Created workflows for reviewing patient information, monitoring health data, managing questionnaires, and accessing relevant patient insights.

Relative App: Designed a simplified experience that allows relatives to stay informed and support the patient's care journey.

Super Admin Portal: Designed administrative workflows for managing users, configurations, and platform-level information.

Across the ecosystem, a scalable UI system helped maintain consistency while allowing each experience to adapt to its specific users and workflows.

The design work helped establish Cordia as a cohesive digital health ecosystem spanning marketing, patient care, clinical workflows, and administration.

The platform supported 100+ patients while providing dedicated experiences for different users and use cases. The scalable design foundation also made it easier to introduce new features and evolve the product across multiple platforms.
