Sparrowell is a HIPAA-compliant web app created to help healthcare providers improve health outcomes in patients with chronic diseases. It offers a platform for population health management, care coordination, analytics, and preventative wellness programs.
Problem Statement:
Providers managing long-term and post-acute care for chronic disease populations face challenges in reducing medication errors, readmissions, and overall costs. Existing tools lack comprehensive, scalable, compliant, and patient-engagement-driven solutions.
