I once let a reply go out unreviewed.
Fine 9 times out of 10, until the 10th time it wasn't. Nothing catastrophic, but enough to change how I build permanently. Speed is good. Speed without a checkpoint is a liability.
→ The fix was simple: one approval step, every time.
