The output is built for a decision, not for theatre: a current-state system and control map; an evidence-linked bottleneck, failure-mode and common-cause register; up to three countermeasure alternatives assessed against explicit criteria and uncertainty; and the smallest sensible 30-day controlled test with a baseline, owner, outcome / process / balancing measures, early-warning indicators, success conditions and stop / rollback conditions.
Scope is deliberately narrow: one workflow, up to three systems or tools, up to ten workflow steps, up to five recurring exception types, and up to three safe supporting evidence items. No call, meeting or live client-system access is required.
