Delivery may use AI-assisted coding under human review. You receive working software that passes the written acceptance test, not an unreviewed AI dump.

• One form, ≤15 fields (text, long text, email, number, date, dropdown) with validation rules you choose (required, patterns like "SO-12345", min/max)

• Public link or staff-only form; spam honeypot; reference number for every submission

• Staff queue: ≤8 statuses with allowed moves, filters on 1–3 dropdown fields, search, detail view, activity log (who changed what, when)

• CSV export of the filtered view (opens cleanly in Excel / Google Sheets)

• Deployed to your own Node host + Postgres (Render, Railway, Fly.io or a VPS), plus a short README

ACCEPTANCE (pass/fail)

1. The form rejects invalid input for every agreed rule and stores valid submissions with a reference number.

2. New submissions appear in the queue; status changes follow only the agreed moves and are logged with who and when.

3. Export of a filtered view has the same row count as the queue and opens without broken characters.

4. Queue and export require a staff login; the wrong password is rejected.

5. Delivered within 5 business days of complete intake and cleared payment.
