Built an End-to-End Clinical Assessment & Lifecycle Automation System for an Aviation Mental Health Practice
Overview
The client wanted more than an online assessment, they needed a complete intake and follow-up system capable of identifying pilots at different risk levels, notifying their clinical team in real time, and delivering personalized follow-up journeys automatically.
I designed and implemented the entire workflow, connecting Typeform, Customer.io, Zapier, Slack, and their website into a single lifecycle system.
The result was an automated assessment experience that routes users based on clinical outcomes, alerts the right people when intervention may be required, and delivers personalized email sequences without any manual intervention.
What I Built
Multi-Outcome Assessment
Designed a branching assessment with multiple clinical pathways that categorized users into four outcome tiers based on their responses.
Each outcome triggered its own experience and follow-up journey.
Lifecycle Email Automation
Built personalized Customer.io campaigns for every assessment outcome, ensuring each participant received messaging relevant to their specific result.
This included:
Personalized onboarding emails
Tier-specific follow-up sequences

Real-Time Operational Workflows
Built automation between Typeform, Zapier, Slack, and Customer.io to create a fully connected operational workflow.
