The buyer had a very technical process to identify health risks within a patient population. I was asked to provide brochure content in a trifold format which explained the concept both simply and with sufficient detail to fully understand the process.

The Alphadelphi Process

Alphadelphi utilizes a medical case management interface called an Electronic Health Record (EHR). This EHR is a cloud-based case management software, which provides access to the information in the system.

Information is input to this EHR based on:

·      Family medical history

·      Patient demographics like age and ethnicity

·      Current medical records

Once this information has been gathered, the platform then uses Artificial Intelligence (AI) to efficiently and quickly identify risk patterns.

Risks are categorized using ICD-9 and ICD-10 diagnostic codes to determine the severity of risk, from minor to most severe.

To determine the level of severe risks, values are weighted.

For risks that are not severe, Alphadelphi automatically distributes preventative educational materials.

A combination of values beyond specified thresholds indicates a more severe risk, and triggers intervention through medical case management.

Medical Case Management

For severe risks, medical case management is triggered, performed by a registered nurse (RN) case manager. Intervention can take place in one of two ways:

Telephonic - the RN works over the phone to coordinate medical care

Field - the RN performs the medical care onsite and in-person

Patients are allowed to voluntarily submit their genetic profile, in order to provide more accurate prediction of risks and outcomes.

The medical case manager utilizes consolidated medical records, which improves the coordination of care, instead of relying on a fragmented set of records from different sources.

This process empowers the:

o   Prediction of risk

o   Tracking of patient care

o   Dissemination of educational materials

o   Coordination of complex medical care

Alphadelphi In Operation

The end-user of the Alphadelphi platform is a registered nurse (RN), who functions as a certified case manager (CCM), to advocate for the patient, and ensure that they receive optimal treatment solutions.

The CCM inputs information into the system, and activates the various features of the EHR system.

The goal is to have the CCM ensure that medical case management provides a smooth coordination of medical care.

This process minimizes delays, and avoids miscommunications.

The CCM identifies providers and practitioners who are most suited to addressed the condition being treated.

By coordinating care in the most effective manner, medical costs are reduced by up to 30 percent and levels of prevention are greatly enhanced.
