Thrivable needed a form that evaluated potential customer needs.
This had a few steps, and each step input determined options for the next step's options. The form was integrated into the home page and needed to follow the page's style to the letter. Every step of the form has built-in guardrails to prevent the user from going forward without selection, every step has a back button and remembers the already selected values.
Each step of the form is indicated by a green circle above the options. Every option in the form then has a tooltip on hover. The form evaluates every single input from all of the steps and when you finish the process it displays custom results based on all of the inputs.
In the end, the form has a script that evaluates if the user is known or not and if he is, without submitting a form using form API to let HubSpot know that he was the user who submitted the questionnaire.
If the user submits the actual form at the last step, it starts an automated sequence of actions in the backend. From letting the sales team know - based on user API the regional sales manager would be notified, to start an email sequence, which utilises already gathered information from the questionnaire.
