This project showcases a user-friendly resident profile interface designed for caseworkers. It enables tracking of alerts, communications, benefits, health data, financial mobility, and family details in one place. Built-in messaging and quick actions (edit, call, message) streamline support and improve resident care coordination.
This interface is part of Ounce, a support platform designed for social workers. It streamlines resident communication, automates processes like Medicaid recertification, manages daily tasks, and provides personalized assistance.
The "Residents" page allows users to view a full list of residents, filter and sort them by name, last interaction date, number of services received, eligible benefits, and personal goals.The interface provides quick access to up-to-date information, simplifies navigation between profiles, and helps coordinate personalized social assistance more effectively.
Ounce is an analytics platform designed to monitor and improve community health metrics. The system consolidates data on resident engagement, care gap closures, Medicaid renewals, and community initiatives. Interactive maps and actionable recommendations help identify problem areas, build partnerships, and allocate resources effectively to enhance quality of life.
