Introduction
Communication is a fundamental part of nursing practice. Nurses communicate with patients when collecting health histories, explaining procedures, providing health education, assessing symptoms, offering emotional support, and coordinating care with other healthcare professionals.
Effective nurse–patient communication involves more than simply exchanging information. It includes active listening, empathy, respect, appropriate questioning, clear explanations, and attention to verbal and non-verbal communication. Research has identified communication as an important component of relationship-building and patient-centred nursing care.
Importance of Nurse–Patient Communication
A strong therapeutic relationship can help patients feel respected, understood, and involved in their care. When nurses communicate clearly, patients have opportunities to express concerns, ask questions, and participate in decisions relating to their healthcare.
Communication is particularly important when patients are anxious, experiencing pain, have limited health literacy, or face difficulties communicating because of language, hearing, cognitive, or other barriers.
A 2024 scoping review examining therapeutic communication in Africa identified several barriers, including patient-related factors, nurse-related factors, environmental conditions, and health-service factors. The review also reported both positive and negative outcomes associated with therapeutic communication.
Key Communication Skills for Nurses
1. Active Listening
Active listening requires the nurse to give the patient appropriate attention and allow them to express their concerns. Nurses can demonstrate active listening through appropriate eye contact, verbal responses, clarification, and summarising information.
Research on communication has associated clinician listening and warmth with greater patient satisfaction, although evidence varies according to the setting and study design.
2. Empathy
Empathy involves recognising and responding appropriately to a patient's feelings and concerns. An empathetic approach can help create a supportive environment in which patients feel comfortable discussing sensitive issues.
A recent systematic review of communication quality during acute hospitalisation found that non-verbal and empathic communication were areas where lower ratings were identified in some studies, highlighting the importance of developing these skills.
3. Clear and Simple Explanations
Healthcare information can be complicated. Nurses often need to explain medications, procedures, discharge instructions, lifestyle changes, and follow-up requirements.
Using clear language and checking whether the patient understands the information can reduce confusion and support patient participation in care.
4. Non-Verbal Communication
Communication also occurs through facial expressions, posture, eye contact, tone of voice, and other non-verbal behaviours. Nurses should therefore consider how their non-verbal behaviour may influence the patient's perception of the interaction.
5. Patient-Centred Communication
Patient-centred communication involves recognising the patient's individual needs, preferences, concerns, and circumstances rather than treating communication as a one-way transfer of information.
An integrative review of registered nurse–patient communication found that communication plays an important role in relationship-building and that communication styles can influence how patients participate in interactions with nurses.
Communication and Patient Experience
Patient experience is an important component of healthcare quality. Evidence from a systematic review and meta-analysis found that nursing interventions involving communication, patient education, nursing rounds, and increased interaction were associated with improvements in several measures of patient experience, including communication about medicines and discharge information.
However, communication should not be viewed as a single intervention that automatically produces the same outcome in every clinical environment. Studies differ in their populations, settings, methods, and measurements. Recent research has also noted variation in how the quality of nurse–patient communication is measured.
Strategies for Improving Nurse–Patient Communication
Healthcare organisations can support effective communication by:
Providing communication-skills training for nurses.
Encouraging active listening and empathy.
Using understandable, patient-friendly language.
Allowing patients adequate opportunities to ask questions.
Considering language and cultural communication needs.
Encouraging nurses to recognise important non-verbal communication.
Using structured communication approaches when transferring important clinical information.
Regularly evaluating patient experiences and communication quality.
Research reviewing communication-skills training interventions found improvements in nurses' communication skills across the included trials, although differences between training programmes and outcome measures made it difficult to determine a single universally effective approach.
Conclusion
Effective nurse–patient communication is an important element of patient-centred nursing practice. Active listening, empathy, clear explanations, appropriate non-verbal communication, and respect for individual patient needs can contribute to stronger therapeutic relationships and better patient experiences.
At the same time, communication quality is influenced by the clinical environment, patient characteristics, nurse-related factors, available resources, and organisational practices. Continued education and evidence-based approaches can help nurses strengthen communication skills and support more patient-centred care.
