For a medical specialist site I'd lean sharp focus every time — motion blur reads as "dynamic" but for surgeons that trust/credibility signal from a crisp, still image probably matters more than energy. Is this a personal call or did the client have input on the tone they wanted?

Motion blur works better here, in my opinion, the CTA could be a bit more prominent

Sharp focus, agreed with the comment below. For a surgeon specifically you want the image to say precise and steady hands, motion blur undercuts that even if it looks more cinematic.
