For each important issue, I document:
Finding — what is not working
Evidence — where the issue appears and what supports it

Severity — Critical, High, Medium, or Low
Recommendation — what I suggest changing
Relative effort — Small, Medium, or Large
I then organize the recommendations into Now / Next / Later so your team has a clear order for what to address first.
