Tasks not uploading to the database

Platform
Browser: Chrome, Firefox, and Internet Explorer.
App: Web app
Steps to replicate:
1, Sign into volunteers.net.
2, Go to create new task.
3, Fill in all fields.
4, Click the submit button.
Expected outcome
A popup that says “Task successfully created” should appear.
The new task should be added to the user’s tasks.
Current outcome
A blank screen appears. Furthermore, we can confirm the task
is not being entered in the database as well.

Impact
CRITICAL: Administrators are unable to set their volunteers
tasks, which means entire organisations are grinding to a halt.
Recommendation: Until the issue has been fixed, all
administrators should forward their task details to Support, so we can manually
insert them via MySQL Workbench.
