Post-Discharge Assistant: Workflow Prototype

Post-Discharge Assistant: Conversational Workflow Prototype I built a conversational demo that looks up fictional care plans and helps users review follow-up tasks. Structured tools retrieve the records, and temporary checklist state tracks confirmations during the session. The public demo uses Python, FastAPI and a browser chat interface with visible tool calls and a bounded model loop. It does not manage real patients, book appointments or write to clinical systems. This is an independent prototype using synthetic data. The case study separates this public demo from the earlier cloud prototype. No clinical validation is claimed.
