Video focus for me. The sidebar is useful, but it competes with the clay hands shot.
I’d choose the layout with the course outline always in view. Having the lesson structure visible makes navigation easier and gives learners a clear sense of progress without distracting too much from the video.

Option 01 for me. Green gets to "care" faster in a hospital setting, while the violet pulls the whole system toward a tech product. Since the identity is AI-first, I'd be curious whether you want patients to read it as clinical or as friendly first.
