A schema rename can pass code review and still break a model or dashboard downstream.
Before merging, check four things:
Was a field removed or renamed?
Did optional become required?
Did a type become incompatible?
Can reviewers see deterministic reason codes, not just a score?

Client: “Can you make this button blue?”
Developer: “Yep, 2 minutes.”
3 hours later:
Developer has discovered:
-a 2019 API
-an undocumented database table
-a forgotten admin panel
-17 console errors
-and a developer who left the company in 2021.
The button is still blue though. 😌
Your third developer just built the third version of the same table.
Nobody did anything wrong. The product grew, the team grew, and everyone solved the screen in front of them.
This is what "outgrown its first design" usually looks like. Not one broken screen, but small differences that add up: three tables, two ways to filter, a button that means something different on every page.
The fix isn't a big redesign. It's deciding once:
→ one table, with every state: empty, loading, error
→ one way to filter and sort
→ a spec your developers can build from without guessing
Then the fourth developer builds it right the first time.
That's normal, and it's fixable.
