Telehealth therapy no-shows: AI agent that preps clients before each session
In behavioral health, telehealth was supposed to make therapy easier to attend, yet some clinics see more missed virtual sessions than in-person ones. Here's how I'd solve it with AI: a pre-session readiness agent that checks the tech a day ahead and helps clients actually get into the room.
In one study of rural Louisiana behavioral health clinics, the no-show rate was 17% for telehealth visits vs 13% in person: https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2807436
How I'd set it up.
1. 24 hours before the session, the agent texts a short tech check: device, connection, and a private place to talk.
2. If something's off, it offers the clinic's phone-only option or an in-person slot. The front desk confirms any change.
3. An hour before, it sends a one-tap join link.
4. If the client hasn't joined 3 minutes after start, it texts and calls once, then puts the client on the front desk's outreach list.
5. The therapist sees who's ready, who's switched to phone, and who's unreached, and decides on follow-up or rebooking.
Any crisis wording in a reply goes straight to the clinician on duty by a fixed rule, not model judgment. The agent doesn't give clinical advice.
PHI stays in your environment, any vendor that touches it signs a BAA, and every message is logged.
Practice owners: is your telehealth no-show rate higher or lower than in-person, and do you know why?
