A revision is the correction of a phase since the previous positive feedback, so it only includes edits to that phase. Example: you agreed on a concept but don't like the sketches, I redesign the last sketches but keep the initial concept.
The feedback rate depends on the estimated lenght of the project. Example: for an express logo (10 days) we can talk almost every three days, for a tailor-made logo (30 days) I can work silently for up to a week.
