Usually, a project for me starts with an introduction meeting, where we discuss the project, instructions, guidelines or any other relevant information. A first discussion is great to come up with a "tactical" plan together, if there are any questions, discuss them. Communication is very important if there are any doubts, most designers don't clarify, but assume when there's something they don't fully understand and the output is not as expected.
After completing the job, a review of the project is required and if there are any changes required, to address them.
