First I gather information about the client's needs and requirements. Based on that, I start collecting references to share with them. Once we agree on a visual path, I start building (designing and developing) the site, page by page, growing out from the core of the site to all the ancillary sections. Iteration after iteration, I don't stop until we're both happy with the results.
