I will create a single page application for you that can have multiple pages with different kind of form inputs. It can have navigation, and event basic authentication requirements if need be.
What's included
Project planning (1 week)
First week will be spent to plan the project with the client, and decide on the functionality and overall navigation of the project. Wireframes will be developed and design and discussed.

A basic flow of the project will be created to be checked, and finalized with the client
