Google Form intake to Google Doc

I was tasked to duplicate a psychology program to use a Google Form as an intake and the output will be a report through Google Docs.

From the Google Form, all entries will be recorded to a database using Google Sheets. Once a row is populated, it will trigger a Zapier task to intake the values in the rows to create a report using Google Docs. A template was created beforehand with the values in the Sheets mapped to the Doc.

The project lasted for two months as the client was unresponsive at the start of 2023.
